If your student (grade 7-12) is planning on participating in a spring sport and they have not already had their PIAA sports physical for the school year, (sports physicals must be on or after May 1st of each new school year) we will be offering physicals in the high school nurses office on Monday, 1/26/26 from 3pm-5pm for spring sports. The cost of the physical is $15 cash only. This is a great opportunity if your child cannot schedule with their PCP before spring athletic forms are due. If your child participated in a fall or winter sport, you do not need to have a new physical completed. Students should wear shorts and wear their contacts or bring their glasses if they wear them. Please make sure to print out the health history form and PIAA physical form attached and bring them along. Parents/Guardians must complete and sign the health history form prior to the student receiving the physical. If this is not complete and a parent is not present, the physicians cannot perform the physical. I've also attached a new form this school year from AHN. The AHN Consent to Treat form gives permission for our athletic trainers to treat your child should they get injured during a practice or game. This form must be completed in its entirety, and both pages must be uploaded into Skyward as you do the health history and PIAA physical forms. Keep in mind, your student is not permitted to participate in the first official practice if this and all online athletic forms are not complete.
